The SAGE Tuition Rewards Program is now available to your children, grandchildren, nieces, nephews & other family members. The best part: This program is being provided to you, absolutely free, by your Financial Professional. Students must be enrolled in the program by June 30th of the year the student begins 12th grade of high school. This deadline gives participating colleges time to recruit potential students (which is why the colleges join Tuition Rewards). Each registered student will receive a 500-Point Tuition Rewards bonus (1 point equals $1.00) upon enrollment. Tuition Rewards are guaranteed discounts for up to one full year's tuition (spread evenly over four years) up to $63,505 as of 2023 - provided by participating private colleges and universities for undergraduate study, beginning with the freshman year. The undersigned Client and the Client's Advisor hereby acknowledge and understand that life insurance policies and/or annuity contracts issued by National Life Insurance Company & Life Insurance Company of the Southwest ("Company") or securities products sold by Equity Services, Inc. (ESI) are purchased as a separate transactions apart from any Tuition Rewards Program provided by SAGE Scholars, Inc. ("SAGE”).
The Company and ESI are acting solely in the capacity of insurer and/or broker-dealer, and make no representations regarding the Tuition Rewards Program which may be provided by SAGE.

By the signature of Client and Client's financial professional below, the undersigned Client and Client financial Advisor acknowledge on behalf of themselves, as follows:

• They have not relied on Company or its agents, General Agents, employees, or representatives, or ESI, and its Registered Representatives or employees for any advice in connection with the Tuition Rewards Program
• Neither the Company nor its agents, General Agents, employees, or representatives or ESI, its representatives and agents have analyzed the Tuition Rewards Program to determine its effectiveness in delivering advertised benefits, nor are any of the above parties authorized to express opinions concerning the effectiveness or soundness of the Tuition Rewards Program
• The Client and his/her Advisor understand that the Company and ESI act solely in the capacity of a life insurer issuing life insurance contracts and annuity contracts or a broker-dealer selling securities' products
• The Client and his/her financial professional covenant not to sue the Company or its agents, General Agents, employees, or representatives and ESI, its representatives and employees, and agree to indemnify and hold them harmless on any claim arising from or in connection with the Tuition Rewards Program provided to Client by third party SAGE
• This acknowledgement and hold harmless shall be binding upon the undersigned and their successors and assigns
